Insurance Policy

Our office staff will be glad to assist you with your insurance coverage.  When you visit our office, please bring your current insurance card.  If you change insurance companies or join another employer’s insurance plan, please be sure to let our Check-In and/or Billing Staff know.  If an insurance referral is necessary, please remember to contact your primary care physician to complete the process on line.  If your referral is not accessible for online processing, please bring a copy with you to your appointment.

We are in-network/a preferred provider for Aetna, AmeriHealth, Blue Cross Blue Shield, Cigna, Clover, , Humana, Independence Administrator, Independence Blue Cross Blue Shield, Medicare, Oxford, PHCS-Multiplans, Qualcare , Tricare, VA Choice, United Healthcare and other assorted commercial carriers.  Please call our office to verify and confirm that we participate with your insurance plan.

Our office will submit all insurance claims for you.  We will fully apply to help you receive full insurance benefits; however, you are personally responsible for your account.    If you have any questions, please feel free to call our Billing/Insurance Department.

Forms

For your convenience, our patient forms are available to fill-in online. Once completed, please print, sign and bring all of these forms with you to your appointment.

Re-Established patient forms must be filled out by patients who have not visited our office in three years or more

The Medical Release Form is provided only if you need to request information such as lab results or patholgy reports from your previous doctor to aid us in providing you skin care. If this information is not needed for your visit, you do not need to complete this form
